I need a conditional multi-step form created. I have nexforms but can't figure out how to get it to do what I want. So, I am looking for someone to build me the multi-step conditional logic form in nexforms (for wordpress) .

It is a 4-step multi-step form. See mockup workflow picture for an explanation of what I need. There are a couple changes from the picture I created (e.g., paypal subscriptions require a different button from paypal for the "subscribe" option)

The requirements are:

1. I need submitted form data emailed to me (nexforms takes care of this)

2. I need form data from submitted form kept in a database table (nexforms does this)

3. I need to insert Paypal buttons for payment on last step (again, nexforms has this capability).

This could optionally be written in PHP, but then I'd have to probably make a custom page in wordpress which i don't want to do. It could be done in javascript, but I will be updating the course list (in the dropdown for Choose a Course select) as time goes by (adding new courses to choose from), and I don't know javascript so I will have a very hard time updating that. I know PHP and HTML and CSS. Mostly, I just want it done in nexforms because that is easily insertable into my theme and my page via shortcode.
