I need our online form updated to include additional fields and functionality.
The form can be found here [login to view URL]
The form needs some additional fields added. the original code for the above page will be provided
The form will collect basic information and have a number of check boxes that will expand to show additional information required.
Based on the services that they click, the form will also add items to a shopping cart for payment (via Paypal) at the next stage.
We want to reduce the initial size of the form and expand for the items that our clients require.
