Scrolling Text Wordpress Widget

I would like a Wordpress widget that operates like the left sidebar does on this site

A clean input interface, which should be located under the Wordpress "Settings" tab, should allow for unlimited entries, and an easy to add and delete interface for individual posts.
The input should allow for 1) a header, 2) the body of the "quote", 3) the name of the quoter, and 4) an optional hyperlink that affects the entire item.
General Options
In addition to a scroll option, I would also like a timed phase-in/phase-out option, with options for a time interval, with a default interval of 5 seconds.
The look and feel of the user interface should be something as clean as WP-Polls 2.31 (just as an example).
The size of the scroll box should be customizable, as well as the speed. I would prefer the defaults to be similar to the [url removed, login to view] example.
I would like this to be forward compatible with new versions of Wordpress, so future support may be necessary.
