Converting an Adobe Illustrator pricing form into HTML

Hi hope everyone is doing well,

I would like to convert an .ai format file into a functional html page. The .ai file is a pricing sheet with a full form, asking the user to fill out their information. This assignment is essentially a one page website that is SSL secured (using HTTPS).

I need an IT professional who has experience in creating online payment forms, as well as facebook add/like buttons, twitter add/follow buttons and linkedin add/follow buttons

See attached for a sample of the .ai file, it should include the following features:

*SSL SECURED*

Forms/fields

Pictures

Check-box 1: Agreement to terms and conditions (instead of the current hand written agreement/signature)

Check-box 2: Willingness to receive communications from the company (newsletters, updates, etc.)

Ability to connect user's facebook account with the company's facebook account (via LIKE or FOLLOW button)

Ability to connect user's linkedin account with the company's linkedin account (via ADD or FOLLOW button)

Ability to connect user's twitter account with the company's twitter account (via ADD or FOLLOW button)

Send button (at the end of the registration)

This is a rather simple task, which I need done as soon as possible. or 24 hours

This should not take more than 2 hours to convert the AI to HTML, as it is only one page ----Experienced programmers wanted.
