Configure Wordpress Plugin called Nice Login Widget - $10 max

I need to configure a login solution for the test login page at [login to view URL]

The Plugin that we will use is already installed and in the correct widget area. It is called the Easy Registration Widget

We need it to show up on the page in question and style it accordingly, using the built-in options of the plugin to do so where possible.
