Appointment Reminder

I am looking to have a script wrote that a user can make an account and subscribe to a package and after payment for the package they choose can login and be presented with a appointment type of system where they can add a client remove a client etc. Then when the add or edit the client they enter the clients name, phone number, appointment date and time, and then select the clients cellphone carrier. And when they want the reminder to go out. By selecting each different carrier I need it to take the clients number and the carrier data and send the client a "email to " so it is phone ,

More data if need after selecting freelancer.

Q- Who will it be sent to?
A- The reminder will be sent to the client in which the person that signed up on the page created.

Q- How will packages work?
A- Set amount of sms sent out, I would like to have email as well so that would be a great extra selectable field.

Q- More about the sms/email
A- If you were to email my cell number in the following format I would get the "email" as a text to my phone I have a huge list for the other carriers as well. [10-digit phone number]@vtext.com So with that said the email should display
[OUR WEBSITE NAME]
This is a reminder from [Person who registered with us]
You have an appointment at |time| on |date|

Q- Payment Method?
A- At this point we plan on using paypal

Question 3 I think should be editable by the person who registers but I should be able to set a default message in the backend/admin area,

Obviously I would need somewhere to monitor and control (suspend, delete, edit, and upgrade members)
