Hi I would like a form to be created using the spreadsheet attached. The form is divided into four 4 groups(Named in the spreadsheet so design accordingly.
The form data should be saved to a database with the result of each calculation.
The calculations are simply 2 values divided as shown in the spreadsheet .In some cases no calculations are needed just straight data entry

Note the form fields must accept numerical values and show error if alpha characters were enered and prevent the form saving data.
The form must be centered
