I want an A4 flyer to hand out at home opens by real estate agents to people visiting the house to possibly purchase.

the back of the form needs to be a form that they can fill out with the information I would need to organise a pre-approval. (I will provide a template of what is required)
Needs to look modern and professional and include my logo and adhere to the colour scheme and design paramaters of my company.
It needs to be in a printable form to send straight to a printer.
