I need a C# .NET web based application that can take a parameter as a query and from that fill in PDF Forms from a MSSQL database.
This will be a simple query where a customer number is entered and relevant information is returned, name, address, city, state, zip, etc?
There will also be another section where a query may be required.
A second section is the medical provider line items on the UB-04 form. A query will be required due to the relation to the customer, as there can be multiple providers.
I will provide all the queries. I am looking for someone to lay the framework for this and I can take it from there.
Not all fields from the form will get populated from the database, these fields will be filled in manually.
Attached are the PDF forms that will need to be filled in.
