In short, what i need is a native android app that enable my user to submit information from the mobile app to the webform (Webform contain a list of fields for submission) instead of user going to the webform directly. Please see attached for further details.

NOTE: If this project is successful, i have another 5 more similar projects. Looking forward for a successful long term engagement.
