Debug my jQuery Mobile app

This is a pretty simple task I believe.
Look at my website
[url removed, login to view]
and click on the first button which says "Get a Ride"
Th rest is just a jQuery mobile form. Everything is in that one file except for a php page which does nothing but print out the POST array (and Hello).
Type anything into the form boxes and submit at the end. The php page accepts the data and prints out something like:
Hello
Array ( [radio-choice-1] => choice-2 [street] => [homestreet] => [homecity] => [age] => [slider] => 20 )
Note that not all of the radio button results are getting through to PHP. What is wrong?
