Create a Excel Field Content Data Export Map

Hello,
I have a websiite where people can fill out information in a form - all the data is stored in fields in the excel sheet - but I have another excel sheet with different field so I have to export the data from one excel sheet which will have hundreds of records and export it to separate individual forms forms to the new sheet. It a Many to 1 relationship with the sheets. I have attached a copy of the sheet where the data must go into to from the sheet that will have many many records. When the script is complete I will have for example 1 sheet with 100 records when the script i complete, I will have 100 individual sheets with different customer information.
