I need some help creating a price tag

We are a baby & kids furniture retail store and looking to freshen up the price tags that we use in store to display our prices on items.

The ideal design would be ONE SIDED and include our logo along website and space for us to fill in the following information for each item. The overall size of the tags would be 5-1/2" H x 3-1/4" W .. they would be rectangular shaped with a slight concave at top along with a circle punched in the top for hanging so please have the design accommodate this

Example of similar design attached from online. Also have attached a copy of our log. We're thinking of printing in black or grey ink on canvas paper or color on canvas paper so please show proofs in this flavor. Logo is attached as well

I have doubt about size. are you mean height=2 inch and width=4 inch?
