IMPORTANT: Your ENTRY should have a link to *your* test server with the requested functionality.

I need a simple HTML5/PHP form which looks very similar to the attached image. The form should allow users to input a comment, and comment title. Users can also respond to a preexisting comment similar to a comment thread. I need the data to be posted on the form immediately (i.e - Page should not postback, no refresh, and I need to see the post immediately) and you should incorporate AJAX/JQuery for this purpose. The "reply" link (see attached image) should automatically open up a text-area below the parent-comment with its own submit button.

Users should be able to delete their own comment. Deleting a parent comment will also delete the entire thread (i.e., sub-comments).

For the sake of testing, create 3-4 users in the database with associated icons/images. The icons/images can be saved in local file. Users should have the ability to login using a username and password on the comment page itself prior to entering a comment.

Backend (mySQL) AT LEAST needs to capture:
BlogID: Just make this an arbitrary, randomly generated GUID
CommentID
ThreadID: this corresponds to sub-comment when user replies
UserID
UserIcon
CommentTitle
CommentDescription
CreateDate: Auto generated
IsActive: Bit Flag (1/0). If user deletes a pre-existing comment, the comment is still stored in the DB but is not visible on the comment page.

Acceptance Criteria: This needs to be uploaded to YOUR test server which I should have access to. . If it works like what I have described, the project has successfully been completed.

Code should be simple, clean, and well commented. This is a VERY simple project which experienced developers can probably do in their sleep. :)
