I have 20+ years of experience doing electronic composition work for a major USA printing company. The graphic designed work that I did for my clients was always 98%-100% correct for their first PDF Proof. I did my own data entry, graphic composing(setting up) of the forms & the proofreading. I emailed Proofs in a PDF format for the client's approval... 99% of the time, the 1st proof was approved and ready for print production.
