I need a lead form created using gravity forms for wordpress, when a user clicks on a button on my website the form should pop up in a lightbox. The form should look cool and should be styled using the same colour scheme as the website. The form should allow the user to enter the following fields: Email - email First name - name Address of Property - address Phone Number - number Is it a house or apartment? - drop down house/apartment how many bedrooms? - drop down 1 to 10 how many bathrooms? drop down 1 to 10 do you have secure parking? drop down - yes/no The form will intergrate with mailchimp where all the info will be stored. I must receive a notification whenever a new lead is generated.
