Finish Bootstrap WebSite (HTML/PHP) - Electronic Form, User Profile

YOU MUST BE TO COMPLETE THIS PROJECT IN 2-3 DAYS! *My client has requested you be from the US, so understand this is not my request* I have most of a website finished for a staffing agency. I need some minor HTML / CSS work, as well as PHP for Forms/Profile Page and this is all Bootstrap integrated. Our Current Design/Functionality: [login to view URL] NEED it to function the same as this site: [login to view URL] But with couple added things. **LOOK AT THE DESCRIPTION UNDER EACH REQUIREMENT** REQUIREMENTS: +Completed in 2-3 days +YOU MUST SPEAK EFFICIENT ENGLISH CHANGES to the website: ++ "Register" new account, and fill out Electronic Form for Job Application/Profile ++++Example: [login to view URL] --> Apply Online (Make the Form exactly the same, BUT Add Username & Password fields to the application so it registers them to database) ++Need a hidden page (NO LINK ON WEBSITE), that will include JUST a PDF file with a download button to download the Professional Employer / Employee Contract. *This is so if someone is hired, we give them employment contract... we don't want everyone to have our Employee Contract. Understand? Basically when we hire them, we will manually send them an email with a link to the site, with a separate page of JUST the PDF, but the link won't be visible anywhere on the site. "[login to view URL]" ++++ Example: See the below attached document - Contract + User Profile (after you Login, should be an option in the header for "My Profile": +++Time Sheet should be a link in the User profile to download a pdf file. ++ USE TimeSheet Document provided, BUT add our logo at the top and Phone Number + Contact Us Form (Footer): +++ Remove the Map please. Make the form - Full Name, E-Mail, Phone Number, Message box + Make the Banner have a high quality dental staffing picture +++ Example: [login to view URL] + Staffing Solutions +++ Change to "Services" , and make sure there are 9 placeholders. 3 rows, 3 columns +DELETE / TAKE OFF THE SITE +++Delete Search Button in the header, Delete "Come to our Offfice" placeholder in the footer, Delete "OFFICE HOUR" placeholder in the footer, Delete All of the social networking buttons in footer, **TAKE out candidate resources, BUT maybe use that page as the blank page for the PDF document** ONLY Thing that should be in the center of the footer is: "CONTACT US" with detailed information. This is a fairly simple project and a good developer could do this is less than a day., I am just very detailed because so is my client.
