We create paper based information forms to log incidents in the areas we manage. We need to develop a hosted web interface where people at different locations can capture the data from the incident report - Basically items such as name and address and then also certain Boolean fields, We then would like to be able to access this data from a administrative back end with some simplistic reporting functions As a second phase we would like to add geo-coding using the google maps api

Hello. Let's discuss at first the workflow. Who can fill the form? Any person or just authorized employees? If any person how are you planning to protect your system from fake form filling? If only registered users then some fileds will not be to fillled (at least name of person who filled form). Next: how many addresses you manage? If there is not so many addresses it will be useful to choose address from select instead of filling it. Will users use mobile devices to fill form? Many questions. But good planning will help to develop good useful system. Let's discuss...
