Create a series of HTML pages / Forms to embed in an existing website and save data to MySQL database

The project involves creating 9 HTML pages that can be integrated into a Wordpress website. I hope that the pages can be “embedded” in one wordpress page so that the forms refresh within the page. Or, the first page opens into an overlay, with the process through the pages then being displayed in the overlay. Im open to suggestions on this though. The screens musts be responsive with the following variants. • Standard web browser view • Tablet landscape view • Tablet Portrait view • Smartphone portrait view The HTML pages will be a sequential series of forms that capture user information, and display basic information back to the user as they progress through the pages using some standard content that will be selected depending on a few simple variables. The data entered by the user will be saved into a MySQL database at each progression from page to page. At the end of the process the user will be presented with a page with the details of what they have added to the database via the forms, plus some “static” information that will depend on one of the options that they have entered via the form. The user will be able to then: a) Print the page b) Save the page as a PDF c) Click through to a payment gateway (the payment gateway will accept the user info and the required payment via a simple API – Paypal or something similar to PayPal) The payment gateway will then return the user to a specified page in the website I will supply all graphics for the screen layouts / UI. I will supply full UX wireframes to explain the user journey before I supply the UI graphics I can supply the UI graphics using any of the following formats: Sketch, Adobe Xd, Adobe Photoshop, Adobe Illustrator. I will provide access to the server and wordpress installation so that the project partner can log in to install and test the work. This project is for a new client who is boostrapping their way to a very early stage MVP proof of concept. I am hoping that if it is successful that it will develop into a bigger long term project. I would want to maintain a successful relationship with the development partner that is selected in order to continue to evolve the project as and when the client can allocate extra funds to it.. Remote working is no problem. I am happy to use skype, trello, slack etc. to facilitate remote working. I expect the project to start by 27 October 2016. I expect the project to take no more than three weeks in elapsed time.
