Create an input type=text field in WooCommerce

I need a PHP Freelancer who knows Woocommerce and Woocommerce before upload plugin.
On the before upload page, I need to set a image on each "Modèle tatouage" and an input text. The customer will type his choice of the body zone, and the text he wrote has to be attached to the order and can be seen from the Order details (in wp backoffice).
There is three products : plan 1, plan 2, plan 3. on plan 1, there is one image and one input text. On plan 2, there two of them, and so on on plan 3.
What i need to do is in the following screenshot :
