Build a small member area table

I am looking for a guy who can program the following member area:
- different clients can login => users are saved in the DB table "users"
- After logging in, client sees a grid with the following information:
Column 1: AZ1
Column 2: AZ2
Column 3: Number of documented findings
Column 4: Invoice amount
Column 5: Due to date
Column 5: Status of case
Column 6: Edit-Button
Column7: Payment Doc-Costs
Column 8: Payment Lawyer
Column 9: realized money
Column 10: Action button
The result shall be able to be searched by "AZ1" and "AZ2"
Furthermore, there is a [url removed, login to view], where client can add a new data row to his user account.
The following information shall be added:
AZ1
AZ2
Link to image 1
Link to image 2
Link to image 3
Number of documented findings (default value = 1)
Invoice amount (default value = 47,60)
By clicking a button "Submit", the following shall happen into the DB.
1. Value of AZ1 is put in the DB field "AZ1"
2. Value of AZ2 is put in the DB field "AZ2"
3. Link to image 1 is put in the DB field "Link1"
4. Link to image 2 is put in the DB field "Link2"
5. Link to image 3 is put in the DB field "Link3"
6. Number of documented findings is put in the DB field "no_findings"
7. invoice amount is put in the DB field "invoice"
8. Timestamp of submitting is put in the DB field "timestamp_begin"
9. "0" is put in teh DB field "status"
10. In the DB field "due_date", it needs to be calculated: End of month of the timestamp_begin + 3 month
(Example: timestamp is 1.9. => content of due date is 31.12.)
11. The DB field "Payment_Doc", "Payment_lawyer", "realized_money" are left empty

As mentioned, in column 10 there are 2 action buttons: By clicking on the action button "Update Status", a popup opens and client can choose the current status out of a dropdown area. By submitting, the popup is closed and value is put to DB. Furthermore, the timestamp of that action is set to the DB field "timestamp_update"
By clicking on the action button "Case closed", the following popups opens:
End of case [textfield1]
Realized Money: [textfield2]
Realized Lawyer: [textfield3]
Documentation: Radio buttons - options:
a. Text 1
b. Text 2
c. Text 3
d. Text 4
By clicking on a button "Close the case", the following happens:
1. The value of the textfield1 is put to the DB field "timestamp_end"
2. The value of the textfield2 is put to the DB field "realized_money"
3. The value of the textfield3 is put to the DB field "Payment_lawyer"
4. In case text1 is clicked, "0" is printed to the DB field "Payment_Doc"
5. In case text2 is clicked, "20" is printed to the DB field "Payment_Doc"
6. In case text3 is clicked, "0" is printed to the DB field "Payment_Doc"
7. In case text4 is clicked, "[amount of the field invoice of the data row" is printed to the DB field "Payment_Doc"
8. The timestamp of submitting the button is put in the DB field "timestamp_final"
Finally, after submitting the second action button, both action buttons shall disappear.
