Build custom php script with mysql databse

I require a custom php script with mysql database that will create printable documents from a form builder. Automatic Folder / File creation system with Mysql Databse Need script to have: Super admin section (Password protected) Super admin can create admins, and users. Super admin can, delete and edit any files or folders. Admin Section (Password protected) Admin can create users, and Add / Edit fields that need to be entered into form creation. Fields can be; Text Box, Text Area, Drop Down, Radio Button, Check Box, Have radio button for Field (Mandatory) beside each field Have radio button for field to select that entry as folder name when script creates folder. Have option to create, add, edit or delete Sub Folders to be created as child folders within new parent folder that script creates Have option to create , select, edit, delete templates for the Document form that the script creates and make printable Option to create multiple documents that the template will create from fields entered in user section. User Section (Password protected) The script will need to create documents that has all the info entered into the (Form creation page) (User section) - the documents created will be from the options selected in Admin section. It also needs to create a folder in the parent directory with whatever field was selected in Admin section for folder name The script also needs to add within the new folder, sub folders from the options created in admin section Users must have options to upload documents and files, pictures into folders (allowable file extensions must be entered in the admin section) **** PLEASE Make sure we communicate and you FULLY understand my requirements.
