Date entry

Work done
Signature
I currently have a good bit of the work done.
I have the signature section complete, and it currently posts the signature to an image in a new tab.
I need to change this, to add the above entry boxes, and once the signature and details are entered, I need the submit button to add all data to an Email and send it to an address.
Doesn't matter what it looks like in any browser but it needs to be for iOS. iPhone/iPad.
Can provide all current HTML, PHP and Javascript files when required.
