I need a spreadsheet for clients to enter data about their sleep patterns, lifestyle and a rating for how they feel about the night they just had. Clients will fill his in each morning while they are undergoing therapy so graphs will be necessary to illustrate progression / regression.
This will include:
Client reference number (manual entry)
Date
Did you take a sleeping tablet last night (yes/no and type)
Did you take any other medication last night? (yes/no and type)
Did you drink alcohol last night? (yes/no and manual entry for quantity)
What time did you eat supper? (hours/ mins in 5 min options)
What did you eat? (manual entry)
What exercise did you do? (start time, duration, type. Type = manual entry)
Did you nap yesterday? (Yes/no, start time & duration)
Agreed “To bed time” (hours /
At what time did you go to bed last night? (hours/ mins in 5 min options)
After settling down, how long did it take you to fall asleep? (hours/ mins in 5 min options)
After falling asleep, for how long were you awake during the night in total? (hours/ mins in 5 min options)
At what time did you finally wake up? (hours/ mins in 5 min options)
At what time did you get up? (hours/ mins in 5 min options)
How well do you feel this morning? (Rating 1-5)
How enjoyable was your sleep last night? (Rating 1-5)
How active was your mind in bed last night? (Rating 1-5)
How physically tense were you in bed last night? (Rating 1-5)
How anxious were you in bed last night? (Rating 1-5)
Other comments (manual entry)
I will need calculations to produce the following:
Total time in bed
Total time awake
Total time asleep
Sleep efficiency (time asleep as % of time in bed)
The following data will need to be shown on graphs (grouped accordingly):

How well do you feel this morning? (Rating 1-5, 1 = Not at all, 3 = Moderately, 5 = Very well)
How enjoyable was your sleep last night? (Rating as above)

How active was your mind in bed last night? (Rating as above)
How physically tense were you in bed last night? (Rating as above)
How anxious were you in bed last night? (Rating as above)
These graphs will show progression over the duration of therapy. Reporting is required in tabular form as well, with options to show daily data in weekly groups (i.e. week 1 or 2 or 3 etc broken down into days).
All reporting to be done in hours and minutes, not minutes only unless duration is under 1 hour. Use 12 hour clock with am/pm option
