Create a pdf form with drawings - Do some Excel Work

Hello All,
I'm looking to convert a quotation form ( paper based currently) into a pdf form that could be filled on an IPAD or Surface , offline. I do have nuance ecopy pdf pro and adobe pro to create the form but I need a drawing section that would give and calculate the lenght according to the drawing. Example to be provided. Could be a pdf, an excel file or other format as long as It can be easily filled on a tablet. important feature is to be able to add a picture from the camera of the tablet ( a lot of free app allow it already)

calculation needs to happen on page 1. page 2 would be the same form but whitout calculation, Page 3 same as page 2. only page 1 in attach file but 2 and 3 are almost identical
need to be able to send page 1-3 via email to the office.
need to be able to send only page 2 to the customer
need to send page 3 only to the production line.

need the work to be done in max 5 days with multiple revision until form is working
