Update Access Database Form and Report

We have an existing Access database that has forms and queries built that generate reports. Need to have it updated with the following changes:
MANIFEST GENERATION FORM (screenshot attached)
1) Add "Resource Request #" - Type: text field
2) Add" Standby (Yes/No)" - Type: drop down - 2 values: Yes, No
3) Add "Date - Arrived at Incident" - Type: text field with date picker
4) Add "Date - First Duty Day" - Type: text field with date picker'
5) Add "Date - Manifest Created" - Type: text field with date picker
6) Add "Marshaling Point" - Type: text field
7) Update "Crew Type" drop down" add "IFR Scan" as an additional option.
8) Rename "Mobilization Date" to "Date - Left Home Base"
9) Rename "Project/Fire#/Stby" to "Incident or Project #" (remove any default value)
NEW TABLE
10) Add a Fire_Centers table. This will have 2 columns: Name, Phone_Number.
11) The "Name" column will populate the "Fire Center" drop down on the manifest generation form. The associated "Phone_Number" will populate the "Fire Center Phone" field on the Manifest Report.

12) Remove 2 rows at the top
13) Add new table at the top (below header). All the fields are shown in the mockup and would need to be auto populated from the tables. I can help explain where each vale is stored n which [login to view URL] fields will be populated and map to the input options on the Manifest Generation Form. Can make the font size, table height and general layout etc the same as the personnel table below. I pasted in the table from excel so looks bigger in the mock up than we want.
14) At the top add a title for the document: "Contractor Personnel Manifest" (shown in mock up)
