An application has to be developed for a municipality in The Netherlands.
The application is to be developed in MS Access because it should be free of any installation issues.
This application is meant to replace a paper form for company environment issues.
The original paper form consists of 23 questions, most of them with more than one sub question. Depending on the given answers one or more questions are sometimes skipped.
The questions consist of a first page with general information about the request and 23 pages with questions.
On the first page a selection is made regarding the categories that apply to the request. Depending on the selected categories (eg. Building, events) one or more of the other paper forms have to be filled in.
Assignment
The application has already been partially developed. Al the sub forms with their questions and validity checks have already been developed.
The problem is to prevent Access from creating a new record for every new sub form opened. All answers for the same request should be stored in a single record in the “re_request” table.
After this the checks for skipping questions can be built in, there is a flow diagram that describes what question to show after answering a question.
Eventually the application should contain a survey with unfinished requests. These requests can be picked up by an employee with the right knowledge.
When all questions are answered a report is generated containing several conclusions.
The conclusions for this report are predefined and can be kept in a separate table.
There are about 13 conditions that make an extra conclusion valid. An example: “If question 3 is yes and question 4 is no and category C, then conclusion T”
