Collect information

I am looking for a program to collect information that has a GUI Screen: This needs to be written in JAVA and saved in a .java files. I need a screen that captures the following information from the users: Name Address City State Zip Phone Occupation with a pull down menu to select from choices Pay Number of years in the occupation calculate Taxes based on the pay as such: 25% of first 10,000, 20% of income over 10,000 but less then 50,000 and 10% of income over 50,000. Also subtract 50 for staff, 100 for supervisor and 1,000 for manager I want the program to catch errors and make users put in the required information I want to be able to save each record to a file, or save all records to 1 file. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform I would like this program to run on a desktop using Microsoft Window XP Pro or Home version, using Java GUI screens.
