Customer Feedback for Wordpress Site

Project Details (Read Entire post before submitting your bid) Brief Over view: I have a service business where my team go to my customer’s home and perform a service. I need my customers to be able to review or rate my team after the work completes and also allow me to view the ratings of my team members. I am not looking for a complex survey. All I need is to ask ONE question (This is an example of what I want + some tweaks) [login to view URL] Details of what I need: • I want a system that will allow me to email my customers directly from my site • The system will allow me to import email address from my wordpress/woocommerce site or import a spreadsheet or add manually. • The system will allow me to send html formatted email • The system will give me the option (via checkboxes) to add/remove to the content of the email. (Options like choosing the names/pictures of the team, etc) • When the customer clicks on link in the email, it will open up the browser and show the customer the smiley survey question. • The customers Reponses will be tracked to their email address and the answers will be saved. • The questions will be a single question – a smiley choice question – see link above • After customer clicks on a choice show comment box for them to leave comments • AFTER the customer submits the survey - do the following o the survey will show a box with thank you message (on same page) for ALL answers o IF the customer is HAPPY with the service show an additional box with links to yelp, google, facebook like…etc . o If the customer is NOT happy with service show a box with text message and discount code for next booking.(This information will be static) o IF the customer is OK with service – then do nothing, just show the thank you message • Each team member will be assigned a value based on the rating choice the customer selects. • Admin side – I need to get notified when a customer leaves a review via email • I need to see ratings of each team member that the customer rated along with comments and rating % ([login to view URL]) It needs to look as shown in the picture. What I use: My website uses Wordpress and Woocommerce with multiple pulgins. Conditions of the project: • What I described above is 90%-95% of the scope of the project. • I need this complete within 7 days. Please bid on this project ONLY if you can do this work AND delivery the complete project within 7 days. • If you delay this project, I will deduct 10% for every 3 days this project is delayed. (Exception to the rule - If the delay is due to additional features I request, then no deduction) • The work has to look clean and professional • The work needs to be responsive – survey should look good in Mobile, Tablet, Laptop, desktop
