Build a Detailed, Specific Web Form

- There would be 5 initial fields for data entry. - There would be 5 radio buttons, the first 2 would be the same just a different label when an email is sent. The next two would resemble the first two. The last one would send a default email. - Once selected, the first drop down menu (YEARS) would appear. - The next field there would be 10 options. This field would be labeled SCHOOLS. - Each of these 10 options would have 1 - 20 options under them. These options change every year. This field would be labeled STUDIES. - Each of these options would have 1 - 3 options under them. This field would be labeled PATHS. - Some of these (not all) would have an additional 1 - 10 options under them. These options change every year. This field would be EMPHASIS. - Some of these (not all) would require an additional option. This option can be dependent upon the EMPHASIS field or the PATHS field. These would be a drop down list of about 70 options that would change every year. This would be the same drop down list used if the third and fourth radio buttons were selected. This field would be CERTIFICATE.
