I am after a customer satisfaction survey designed that i can send out via email to customers after they have purchased from my online store. It needs to be a survey that is contained in an email that my CMS system will automatically send out, the questions that need to be asked in the survey are (the software needs to allow me to change these questions) contianed in this link (we currently use Survey Monkey but it looks crap) [login to view URL] My website is [login to view URL] and the email design needs to reflect this with the same colours and logo etc. Importantly the data needs to be captured such that i can analyse it easily (excel or online) I have attached our logo and the colour (RGB) guide.
