Additions to CVTD Form(repost)(repost)

1. The form does not seem to be working right now. We just need to have it checked for repair 2. We need to add an email function that sends the comment in an email to certain addresses. [login to view URL] are getting a lot of form spam so we need one of those validators to avoid that. 4. We need a date stamp on submittal 5. Some of our search categories are not working. Thanks
