Add my opt-in form html code into ready made squeeze page and create separate opt-in form from the one included in the ready made squeeze page

Hi, I have a ready made squeeze page (you can view it at <[login to view URL]> ) this page has three opt-in boxs which are to? link to my auto-responder, when the visitor enters their name and email address and clicks the 'get my free report!' button, their details are sent to my auto-responder which then sends them an email with their confirmation link, once they confirm, an email series begins, each email is supposed to be personalised with their name. I have tried entering my auto-responder code myself and although this allows my visitors to subscribe to my news letter, my auto-responder emails to them are not personalised so their first name data is not being captured, my raw opt-in form code itself does work correctly, but when I trim it down to make it work with the squeeze page opt-in form, only the email address is being captured. So first of all? what I want is someone to enter the relevant code from my basic auto-responder opt-in form? in the three places in my squeeze page so all three opt-in boxs on the page work correctly, that is capture my visitors first name and email address. And second, I want a stand alone opt-in box creating, the same as the ones you see on my squeeze page, including my auto-responder? opt-in form code, so I can just use the opt-in box on its own without the squeeze page. I will provide you with the raw opt-in form code as generated by my auto-responder, the [login to view URL] document which is my squeeze page and a folder with all the images used on the page, anything else you need?
