Customize Fields in OpenCart Checkout and Admin Page

I need someone to help me in adding new fields in my OpenCart Checkout Page.
The site is a new florist webstore, using the latest OpenCart version 1.5.6.4.
The new fields are not provided by the OpenCart default installation, they are as follow:
- Delivery Date (Date format, ex: 30/06/2014)
- Delivery Time (Time format, ex: 10:35)
- Addressed To (text format)
- Template Message (Dropdown preformatted message, ex: Happy Wedding, Congratulation, Deep Condolences, etc)
- Custom Message (Area Text format)
Actually I need the similar fields as well in the Admin area for:
Actual Delivery Date, Actual Delivery Time, Recepient Name
They are not exactly the same as above, since the one in Admin area is for the reporting purpose.
So the idea is like this, in the Checkout page, the customer will request the date&time for delivery, whereas in the Admin Area the admin will input the actual delivery date&time and recepient , which will be displayed in the Order History page of the customer.
