Create a simple one page form app

You will be required to replicate a form , its a very simple one page app but you must be able to finish the project in 24 hours. You will be developing the app in FREE ZOHO CREATOR. It is a proprietary language, unique to the Zoho development environment; it is called Deluge. It has properties similar to PHP, JAVA, and, HTML. The other language that will be used is HTML 5 with CSS.
If you know your stuff you should be able to complete this task within 5 hours. Please note this is for testing purposes only for long term hiring. This is not a real project but you will be compensated for this. Again, this is just to test your skills .. this is how the form will look like... but will send you which one if you are able to do the project.
Please note it must be done in FREE ZOHO CREATOR.
