Contact form amendments on medical website.

I have a simple html website (medical services) which needs the contact form amended.
These are the basic amendments:
'Service required" - needs to have a drop down menu for the services.
Appointment times - should be able to select a date from a 'pop-up' date option.
Thats it.I need someone to do it now.. asap.
