cleanup/fix code on 3 page order form

The site is not working right now. I did some work to change some functionality and design. The code needs to be fixed. Some problems: The storenumber text box shows the store number not name after logging on as super user, selecting company does not work After logging on as super user, submitting gives an error Do not use a form within a form EMSStoreNo is not unique, so you need to use ID (new field) This site was working except for problems with selecting using a field that was not unique and for submitting the order using superUser (form submit would do nothing) A nested form tag was used which is bad design. If you see a change and do not know why it is there, please ask before changing.
