Create mysql database from xml file

I need the following done:
1. Create a new database from the content of an XML file. That is, the structure of the database.
2. Create a c# code that can fill in the data from the XML file to the mysql database. I must be able to build an exe file, that can execute the update.
I have attached an example of the xml file. All data must preferably be in one table, exept the field PostCodeIdentifier, which can be in another table (multiple lines)
