Create an iPad / iPhone App

We need to create a relatively simple ‘Stakeholder Engagement App’ optimized for iPhone (primarily), ipod touch, and iPad (secondarily).
This app will be similar in function to the Borealis IMS App (for iPhone and iPad and android) and Stakeholder Tracker App (for iphone) with the exception that instead of syncing to an existing databse, a report is generated which is then emailed to the app user and or their contact(s). The end report must be able to copy-and-paste from the email to an excel spreadsheet.
The app must be able to:
- Keep list of contacts identified by project and give a warning if the same person name is imported more than once
- Be able to enter data into a form that upon completion is then emailed to the user and/or one of their contacts
- Generate form report in a spreadsheet format that is very easy to copy and paste into excel
- Data to be entered by combination of : ‘select from available list of options’ ; check boxes and blank box form entries
- Ability to save/store current data on form if user is interrupted during entry so they may return to it at a later time
- Able to attach photo(s) to send with email as well (not part of generated report but as attachment to email)
- able to enter blank form data via voice dictation using microphone feature on apple iphone/ipad keyboard
- able to attach location (gps coordinates or google maps location from location enabled phones) at check of box
- user must be able to select language of app - English, Spanish and French options
Please let me know how many hours you estimate it will take to complete the project upto/including submission to the Apple Store. Please also provide a link to a sample of work that shows your simple, clean, flat design style, and tell us in 2-3 sentences about your experience.
App names, logos, content, and template forms will be provided by us. Bu we would welcome design help if you have the skills.
Please also let us know if you have the capacity to create this app for both android and blackberry as that will be a subsequent (phase 2) project.
