Add a calender to a HTML5 website - repost 3

Hi Freelancers, I am looking for someone with sound understanding of HTML5 and jQuery to add a calender to our site. How the calender works: users can pick dates from the calender (current date or past days) and fill a survey related to those date. The calender is going to show which days are missed and for which days the survey has been filled. We are going to convert this site into native mobile applications so it would be advantage you use jQuery compatible for mobile app. The winner of this project must have a basic understanding of JavaScript (Plus CSS and HTML).
