Access Database must have the following fields
First Name, Last Name, Address, City, State, Zip, Phone Number, Cell Phone Number, Emergency Contact, Emergency Contact Address, Emergency Contact Phone, Notes, Plan Selected.
The Plan Selected will need a drop down box:
One Call Bi Weekly
One Call Per Week
Two Calls Per Week
One Call Working Days
The Invoice will be based on these plans
1 Call Bi-Weekly 5.00
1 Call Per Week 7.00
2 Calls Per Week 14.00
1 Call Working Days 45.00
Setup Fee 30.00
The Setup Fee will be a one time for the first invoice only.

I offer you access application with very user friendly forms having functionalities like auto filling easy data selection through dropdowns, Search data, Validation rules, Filter some data then save the filter with a name so you don't need to filter for future use.
The user interface structure of the application will be such that there will be a single Home form and all the other forms can be loaded on this main Home form through button clicks to access their functionality.

I will provide you with a solution to manage plans and invoices. The DB will contain all the contacts along with menu driven forms to maintain these contacts, associate contacts to plans, and send invoices according to these plans. I will provide you with some extra forms to view due invoices, paid , etc.
