I'm trying to create a dynamic form that allows me to add unlimited categories and unlimited questions under each category. Upon submit I would like to be able edit values/display errors per question. I'd like to dynamically add new categories to a database, and have them dynamically appear in the categories dropdown. An example of what I want is at [login to view URL] Please only bid if you are sure you can complete the project.

Hi, We have reviewed the referenced form and can create a similar one with unlimited option of adding category/question. Adding category dropdown directly from database will be a tough ask but its achievable. Looking forward for your response. Thank you.
