ASP.NET/MSSQL/Javascript Developer Needed

I'm looking for a freelancer, that can develop a web form to calculate some fields and save one field into a database table.

Sample codes of previous work and a video that shows user interaction can be found in attachments. You must watch the vide, review the code and tell me that you guarantee that you will deliver what we want to do...

I'll provide the user manual if needed, but you must work on Teamviewer, side by side with me...

I'm expecting 3 hours of work. I need someone to deliver the work quickly and start as soon as possible (today).

I'll only escrow funds to those with good feedback. Otherwise don't ask for a prepayment. I'll not pay more than 30 USD for the task. So bidders requesting more than that will be simply ignored.

I'll accept the work only if code works and results are approved by my supervisor.

There will be future opportunities for successfull candidates
