Design a Flyer

I want you to design my membership form. I attached something that I want it to look similar to (but not the exact same, I don't want you to copy.) I just want you to have an idea. I want there to be green in the form to match my logo. Also, I want an illustrator or psd file and pdf please. Here is what I want you to include: The logo somewhere in the top which I will attach. I also want Membership Form in the top as well. A class will be deducted if you purchased a 5 or 10 class pass. -All the money made from the late cancelations and no shows will be added to the Giving Back Yoga fund and donated to charity.
