Build a Truly Responsive Web application with MySQL

I need a mobile Web Application. I also need a website for administration of the application business data. ********* Your responsabilities as developer are as follow: - Design an implement MySQL database to store application and website information. - Create a beatiful UI design for the WEB application. - Implement the web application and the responsive mobile web application. - Deliver very well documented source code and source project for the Web application. - Design (UI) and implement this beautiful website application for administering application business data. ********* The responsive WEB Application functionality is as follows: It has FOUR roles: Customer (mobile app user), Agent (mobile app user), Clerk (Website user), Administrator (Website user). Mobile WEB Aplication details: - Screen for selecting between two roles (customer, agent). - When role customer selected -> Customer functionality: .Create profile and get application user and password by mail. The profile has to be verified and approved by the Clerk in the website. .Once logged in, the Customer has the ability to see all his "insurance policies" ex: Life insurance, automovile insurance, etc.. Each "insurance policy" registry has 10 to 20 data fields. .Receive email notifications: [login to view URL] the "insurance policy" is about to expire (for example 5 days before. This duration should be parametric in the MySQL database for each customer and shoul be configurable by an admin in the web application). 2. when the next payment date is close (for example 5 days before. (each insurance policy should be associated to a list of payments). - When role Agent selected -> Agent Functionality: .Login to the mobile web application. To be able to login, the Agent should have been previously created by the Clerk in the Website. .Once logged in, the agent should be able to create an insurance policy Quote Request, typing between 10 to 20 fields. Once the Quote is requested it appears in the Website and the Clerk creates the Quote (outside of the system) and enters its data in the Website. The Quote then appears to the Agent as a push notification and its information is available in the app. Web application functionality: - Login for Clerks and Administrators. - CRUD for Clerks data. - CRUD for Customers and its insurance policies data. - CRUD for Agents data. - Approve Customers profile request. - Browse for Quote Requests. - Enter Quote information. - Reports for Quote requests made by all Agents and Quotes by Clerk. ******* Important information: - application UI language should be parametric.
