Design a 'Get A Quote' section for a website

I do not need a whole page designed. I need one section of a page designed. I need a 'Get A Quote' Section. It needs to be visually appealing. I already have developers who can make a form on a page. I need a designer who can make it visually stunning, whether this be through layers, 3d effects, backgrounds, shadows, icons, different shapes, patterns.
The form has 3 questions:
Question Type Visible Answer
1. “Cover Required For?” Radio button Yourself
2. “How Much Cover?” Drop down menu £100,000
3. “Cover Period?” Drop down menu 20 Years
Button – “Get Quote” or “Next Step”
P.S. This shouldn't take more than 2-3 hours of your time. No more than £60. For the artists out there, this is piece of cake.
