Build a Horse Racing Information Website

I need a website built and designed with a modern look and feel. This is for Horse Racing Information Service. I have a logo that I would like to include somewhere if possible.

It will have just 5 pages, 4 of them static. However we will require one page with user input required. We will need to offer a login to the website option. Also the option for user to make change to one page and their results be remebered and displayed everytime they login to the website. This information should be stored in a MS SQL database (Not MySQL)

This must be coded using HTML with a MS SQL backend database and it must run on my web host server, which is MS Windows (NOT LINUX).

MAP Page:

This is where all user interaction takes place, needs to show a MAP of UK. Need to show list of venues showing a red light, that has the ability to change to green light with user interaction. when link is clicked open us an information box showing:

Fixtures:

Google maps:

Link to facebook to create an event:

I need to be able to amend this in future if extra venues need to be added or removed. Please provide instructions.

Code must be provided and tested on my server before final payment is made.
