Design a vector artwork t-shirt transfer for church

I need a T-shirt transfer designed. It needs to be vector artwork and fit in an 11 inch by 11 inch area. The transfer vendor I am using has some requirements for vector artwork found here: [login to view URL] . By accepting this job, you agree that all copyrights will belong to me. I need one white and one black design. In both designs the red portion will remain red. I would also like a jpeg rendering made available along with the vector art. I need .ai and .jpg formats. I would also like a rendering of the design on a light blue T-shirt. A big design on the back of the shirt and a small design in the upper left chest of the front of the shirt. The attached picture shows the general concept. The words on the design are as follows: top row is "Nardin Methodist Church", middle is "Reaching Up", and bottom is "Reaching Out". Feel free to move the words around to fit the best. The font for the words I would like to be "Birth of a Hero" available on [login to view URL] . Please use your artistic ability and come up with any cool ideas you have.
