We need to create a google form for our standard report. Important is the output. We need to have the output structured in a way so that each person filling out the report is listed in the output file seperately but the answers are listed in a more structured way as compared to the standard output. Moreover, the answers including the questions should be sennt to the person filling out the file via email.

Dear Sir or Ms. I am John, pleased to meet you. I'm well experienced in Google Apps Script and Google Docs. If you please, take a look at my portfolio in my profile or here [login to view URL] It is not possible to add bullets inside a spreadsheet cell. But I think there is another more good solution, once the form is submitted the information could be copied in a Google Doc and formatted exactly as you want. This file will remain in your drive and a copy will be sent as attachment in pdf format to the user that filled the form ( if the email is known). In the original spreadsheet, we can add a column with links to the doc files. Thank you for your time ! John
