Adobe Livecycle Form Enhancement

Summary: ============ We have a form that requires the following updates: 1. Some formatting updates, including conditional formatting 2. having two other forms inserted into it. The new forms will only show up in the intake form if the users indicates that it should be shown (through a checkbox or radio box). 3. The 2 forms being inserted (< 1 page each) require some formatting and conditional formatting updates. 4. Please apply your graphic design/form layout abilities to make the form look great. Attached is a video displaying the main form that requires enhancement. Please watch carefully before quoting. The two forms that need to be inserted are attached here as PDF files Line by line changes required are detailed in the "Detailed Project [login to view URL]" file. ## Deliverables Project structure: ============== 1. We would like to start as soon as we find a bidder with an acceptable proposal. 2. We would like to have a 1st draft ready within 2-3 days of the start date. 3. At the first review, we will meet over skype. Expect to spend 1-3 hours together reviewing the form and making any tweaks to the form as requested (Likely formatting changes). 4. If the tweaks cannot be finished in that meeting, we *may* need a second meeting the following day. (Ideally this would not be required) 5. We will test the form with our users for 1 - 2 weeks. We will send a list of changes. Expect minor formatting changes. Major changes (such as inserting a new section) can be subject to an expansion in scope/$. 6.
